POH Youth Care

A POH-youth (youth practice assistant) at our practice provides support for psychological and educational problems in children and young people through consultation, screening diagnostics, short-term guidance and referrals.

The POH youth provides accessible assistance, relieves the burden on the GP, helps with early identification of problems, and can ensure rapid referral to appropriate specialized care in youth care or mental health care.

Our practice works together with the Center for Youth and Family Central Limburg.

The tasks and role of a POH youth:

  • Consultation and triage:

The POH-youth helps the GP to assess the severity of mental health complaints in young people and their families.

  • Screening and diagnostics:

The POH youth can conduct a comprehensive intake and screenings to clarify the nature of the problems.

  • Short-term guidance and treatment:

The POH-youth provides short-term support, psycho-education and parenting support within the general practice.

  • Reference:

After an intake and assessment, the POH youth refers to appropriate specialist help.

  • Low-threshold care:

This support enables children, young people and parents to quickly receive the right help in a familiar place, which lowers the threshold for care.

  • Relief from the GP:

The POH youth relieves the burden on the general practitioner by addressing specialized youth problems in the practice, which allows the general practitioner to focus on other tasks.

  • Cooperation:

The POH-youth works closely with the general practitioner and other care providers to ensure an integrated approach for the child and family.
