POH Somatics

A somatic practice nurse (POH-S) guides and supports patients with chronic physical conditions such as diabetes, cardiovascular disease, asthma, and COPD. The POH-S collaborates with the general practitioner, performs checks (such as blood pressure and blood sugar), provides information and lifestyle advice, and coaches patients in managing their illness and medication use.

 

Tasks of a POH-S

  • Support for chronic conditions: The POH-S supports patients with chronic diseases, such as diabetes, cardiovascular diseases, asthma and COPD.

  • Regular check-ups: Periodic checks take place, during which the POH-S uses measuring instruments to check, for example, blood pressure and blood sugar levels.

  • Information and education: The practice nurse provides patients with information about their illness, the treatment and how they can cope with it in daily life.

  • Lifestyle advice: Attention is paid to lifestyle factors such as nutrition and exercise, and to medication use.

  • Smoking cessation: The POH-S can provide support in quitting smoking.

  • Coaching: The practice nurse coaches the patient in managing their illness and achieving health goals.

  • Elderly care: The POH-S can also provide support and guidance to (vulnerable) elderly people.

 

Method

  • Collaboration with the general practitioner:

    • The POH-S works closely with the general practitioner and provides care under his or her supervision.

  • Own consultation hours:

    • The practice nurse has her own consultation hours and can also make home visits.

  • Reference:

    • Patients can be referred to the POH-S by their GP or contact them themselves for an appointment.

  • Regular conversation partner:

    • For patients with chronic conditions, the POH-S is often a permanent and accessible discussion partner.

Costs

There are no costs associated with a consultation with the practice nurse (POH-Somatiek). Please note: some tests, such as laboratory tests, may be charged to your deductible.
